Grades Preschool, K - 8th August 20 & 21, 9:00AM-3:00PM
If you are attending Guffey School, but can not make these dates, please call the school so that we can plan a place for your child. (Entrance age requirements for kindergarten are any child may enter if five years old on or before October 1st of the year of enrollment).
New Students Will Need the Following for Registration
- The child's social security number or card;
- A birth certificate, or other proof of age;
- An up to date record of immunizations as required by Colorado law;
- Addresses and phone number(s) of the child's doctor and dentist as well as of relative or friends who can be contacted in case of emergency.
Required Immunizations
Colorado law required these immunizations for children age 5 and over:
- DTP - grades K - 12 (5 - 18 years) 5 doses unless the last dose was after 4 years of age then 4 meets the requirement;
- Tdap - required for all incoming 6th graders unless the student has had a Td within the last 2 years;
- Polio - same grades and age - 4 doses unless the last dose was after 4 years of age then 3 meets the requirement;
- MMR - 2 doses K - 5th grade and 7th - 12th;
- Hepatitis B - same grades and ages - 3 doses;
- Varicella (chickenpox) - The second dose is required at kindergarten entry. One dose is required for grades 1-7. It is now required that reporting of chickenpox disease be documented by a healthcare provider. All children new to the Colorado school system and all children who have chickenpox disease that has not been recorded on the Certificate of Immunization prior to the 2007-2008 school year must now have documentation by a healthcare provider or laboratory confirmation showing immunity to the disease.
New students who do not comply have 14 days to comply or will be sent home from school. An exemption form is available for medical, religious, or personal reason.
